Take a look at the different services and support groups available to our patients at Heeley Plus PCN. Each service will have information on how to get involved or support.

<strong>Alcohol & Drug Support</strong>

Alcohol & Drug Support

The network offers support for it’s patients for drug and alcohol related problems. Currently in Gleadless we are offering drop in sessions for our patients in the network if they wish to attend.

<strong>Chronic Pain Group</strong>

Chronic Pain Group

We offer group sessions for patients experiencing Chronic pain. These are led by GP and Health coaches in the network and cover education, understanding the biological and cognitive mechanisms of pain and provide a safe space to talk about challenging issues around living with pain. Your pain is very real and we understand this. The good news is that you are not alone. Learn to understand pain and listen to stories from people with decades of experience living with pain who are now living fulfilled lives.

The pain café is lead by Lee Vaughan our lived experience expert with the support of medical experts and GP’s across the network, you can read his own story here.

<strong>Diabetes Roadshows</strong> <strong>& Education</strong>

Diabetes Roadshow & Education Sessions

The diabetes support café (Diabetes Roadshow) is for patients with Type 2 diabetes. This is run by our peer support volunteers & our clinical teams, we run an hour and a half session at a lunchtime in our surgeries around the network.

<strong>Enhanced Access Hubs</strong>

Enhanced Access HUBS

Patients registered in one of our practices can access an evening or weekend appointment by asking for an enhanced Access appointment. Doctors, nurses and pharmacists all work these clinics, enquire with your GP practice to arrange an appointment.

<strong>Link Workers</strong>

Link Workers

Link Workers are non-medical support staff who focus on a ‘What matters to me conversation’ with people, they help to create a plan together and provide an holistic approach to health and wellbeing. Connecting people to local services, resources and community groups, they find what suits their needs and help with practical and emotional support.

Health Coaches support people to identify how they can implement sustainable health and lifestyle changes in a way that is meaningful to them and benefits their health.

<strong>Mental Health Assessments & Treatment Support</strong>

Mental Health Assessments & Treatment Support

Mental Health Assessment and treatment support for our patients provide treatment and support for patients living with serious mental illness. It is the opportunity for a patient to have a telephone assessment with a mental health practitioner from the primary care mental health team.

<strong>Pharmacy Services</strong>

Pharmacy Services

The pharmacy team consists of prescribers, or completing training to become prescribers and work alongside the practice team. They help with care management of patients with chronic diseases and do clinical medication reviews to proactively manage people with complex polypharmacy. Pharmacists have specialist expertise in the use of medicines to help address both the public health and social care needs of patients at practices and to help in tackling inequalities. They lead on person-centred medicines optimisation and quality improvement.

Through structured medication reviews, we support patients to take medications to get the best from them, reduce waste and promote selfcare.

<strong>Physio Services</strong>

Physio Services

Our PCN physios are triage based physios helping to guide patients on the best available options for their musculoskeletal issues, they predominantly offer face to face appointments but also have capacity for telephone appointments.

They provide patients with advice, education, home exercises, possible lifestyle changes and signpost to relevant services or information to help patients manage their musculoskeletal problems effectively. If required physio’s can arrange certain investigations for patients where appropriate.
